Calcineurin-NFAT dynamics correspond to cardiac remodeling during aortic banding and debanding, mimicking aortic valve replacement

چکیده
Aortic valve stenosis (AS) is a major health problem. Extensive myocardial remodeling increases operative risk and might lead to incomplete reverse with persistent symptoms after aortic replacement (AVR); this makes the optimal timing of AVR clinical challenge. The pathogenesis behind unclear. Central among signaling pathways in heart pro-hypertrophic Ca 2+ -activated calcineurin its downstream nuclear factor activated T-cell (NFATc1-c4) transcription factors. We investigated calcineurin-NFATc dynamics patient mouse hearts during remodeling. Myocardial biopsies were obtained from AS patients left ventricles harvested mice subjected banding (AB) debanding (DB). transcript protein NFATc-responsive gene regulator 1-4 (RCAN1-4) luciferase activity NFAT-luciferase used as read-outs for activity. Calcineurin-NFATc activation was sustained through AB 24 h 18 weeks elevated patients. All four NFATc isoforms AS, while NFATc4 persistently chronic mice. NFAT remained reversible when 1 week’s followed by week's DB accompanied functional improvement. However, week 4 weeks, not reversed. In conclusion, calcineurin-NFAT correspond cardiac experimental DB. Our data suggest that attenuation important outcomes AS.

Transcatheter Aortic Valve Replacement: A Review Article
Transcatheter aortic valve replacement (TAVR) is a novel therapeutic intervention for the replacement of severely stenotic aortic valves in high-risk patients for standard surgical procedures. Since the initial PARTNER trial results, use of TAVR has been on the rise each year. New delivery methods and different valves have been developed and modified in order to promote the minimally invasive p...
متن کاملAortic Annular Enlargement during Aortic Valve Replacement
In the surgery of aortic valve replacement is always attempted, as much as possible, to implant the larger prosthesis with the mains goals to enhance the potential benefits, to minimise transvalvular gradient, decrease left ventricular size and avoid the phenomenon of patient-prosthesis mismatch. Implantation of an ideal prosthesis often it is not possible, due to a small aortic annulus. A vari...

Although classic and standard procedure for patients with ascending Aorta aneurysm with Sever aortic Regurgitation is Bentall surgery but in some selected cases this complex surgery was not performed with different reason like advanced age and coexisting comorbidity involving renal and hepatic and lung Function. we report a rare case of acute right coronary artery stenosis developing in a patient undergoing aortic valve replacement. we present a case report with a brief overview of the literature relating to coronary artery occlusion associated with cardiac valve surgery – the theories and treatments are discussed. a 85 year-old female was admitted under the care of the cardiothoracic team with signs ...
متن کاملAbundant dystrophic calcifications mimicking aortic valve abscess in a patient undergoing elective aortic valve replacement
Dystrophic calcifications of the aortic valve may cause symptomatic aortic stenosis and account for a significant portion of patients who undergo elective valve replacement. Calcifications appearing grossly as a cloudy fluid surrounding the aortic valve leaflets are an uncommon finding. Normally, calcified aortic valves are characterised by large, nodular masses within the aortic cusps. We repo...
